Lori Yoder, LCSW, was a key contributor to the successful launch of the Collaborative Behavioral Health Program (CBHP) at Northwestern Medicine. She served as the first Behavioral Care Coordinator, post pilot phase, in launching CBHP into NM Primary Care at an offsite clinic. Ms. Yoder also supported the sustained launch of CBHP into all 13 Central Region (City of Chicago) clinics, finishing the launch during the Covid 19 pandemic. Ms. Yoder supervises the 6 Behavioral Care Coordinators in CBHP, as a manager within Outpatient Psychiatry, and collaborates with all NM Primary Care leadership to maintain and grow CBHP. Most recently, Ms. Yoder worked closely with NM Data Analytics to launch a robust CBHP clinical data dashboard. Ms. Yoder has a background of providing clinical social work services and manager support in public education, community mental health, and healthcare. In addition, she has taught as an adjunct professor in several social work graduate programs in Illinois.
