Clinical and epidemiological characterization of candidates to heart transplantation using the Stanford Integrated Psychosocial Assessment for Transplantation(SIPAT)

The standarized evaluation of the psychosocial factors in candidates for transplants is of great relevance given that these can affect the outcomes. While the assessment of the medical factors influencing said outcome are well standarized, there has not been a similar strategy stablished for the psychosocial elements. Given the importance of the latter, the Stanford group developed a standarized assessment, the SIPAT (Maldonado, 2012) that was later validated in Spanish in 2019 (López-Lazcano, 2019). The main objective of this study is to present the clinical-epidemiological characteristics of a Spanish population of heart transplant candidates that were evaluated in the Cardiology Service of the Hospital Clínico of Barcelona(HCB)

A retrospective review of clinical data and an interview including the SIPAT,was performed in heart transplant candidates from 01/2019-12/2022. Then the data was entered into a database and statistics analysis were done using SPSS. The data recollection for this study was made in agreement with the HCB`s ethical requests.

55 patients met the inclusion criteria. The mean age of the sample was 53,5 years, being males 69%, with a predominance of caucasian ethnicity (94,5%). 60% of the candidates were married, 63,6% had a secondary level of education, and the occupational status was inactive in a 60% of individuals. The mean SIPAT score was 11,75 (SD 7,9). The patients were categorizes into 3 according to their SIPAT score (excellent, good and acceptable candidates) 10,9% were acceptable candidates, 54,5% were good candidates and 34,5% were excellent candidates. When the sample was analized according to their clinical characteristics: 69,1% had another disease, different from the one that originated the transplant, and 30,9% had a history of a psychiatric disease, mostly anxiety (12,7%). There weren't any statistical significant differences between the different candidates SIPAT categories in relation to the civil status, ethnicity, gender, study levels, number of Emergency and Medical admissions.

The mean SIPAT score found was similar to the results of different international series, with range goes from 12,9-20,89. Up to a third part of the sample was categorized as excellent candidates. In the literature this ranges from 5,5%-24.8%. The rate of psychiatric history found in the sample is higher than the 24% described in the international literature when heart transplantation candidates are considered.(Takano, 2023)

The findings in the Hispanic population are similar to other samples from different populations worldwide, so it could be said that SIPAT is a robust tool no matter the language used.
